Media Release & Liability Waiver

Effective Date: July 30, 2025

By registering for, attending, or participating in any Motion Collective Dance, Inc. (“Motion Collective,” “we,” “our,” or “us”) classes, programs, workshops, events, or activities, you acknowledge and agree to the following terms.

Media Release

​

I grant Motion Collective Dance, Inc. permission to photograph, video record, and otherwise capture my image, likeness, voice, and movement during participation in classes, rehearsals, workshops, performances, and events.

I understand and agree that such media may be used for:

• Promotional and marketing purposes

• Website and social media content

• Educational and instructional materials

• Archival and documentation purposes

I acknowledge that:

• I will not receive compensation for the use of such media

• Media may be edited and used across multiple platforms

• All media remains the sole property of Motion Collective Dance, Inc.

If I wish to opt out of media use, I understand that I must notify Motion Collective in writing prior to participation.

 

Assumption of Risk

I understand that participation in dance, movement, and physical activity involves inherent risks, including but not limited to physical injury, muscle strain, falls, or other bodily harm.

I acknowledge that:

• I am voluntarily participating at my own risk

• I am responsible for monitoring my physical condition and limits

• I will modify movement as needed and take breaks when necessary

Liability Waiver & Release

​

To the fullest extent permitted by Georgia law, I hereby release, waive, and hold harmless Motion Collective Dance, Inc., its directors, officers, instructors, staff, volunteers, affiliates, and partners from any and all claims, demands, injuries, damages, or liabilities arising from or related to my participation.

This release includes, but is not limited to:

• Personal injury

• Illness or medical conditions

• Loss or damage to personal property

Medical Responsibility

​

I affirm that:

• I am physically able to participate in dance and movement activities

• I have consulted a medical professional if necessary

• Motion Collective Dance, Inc. is not responsible for determining my fitness to participate

Personal Property

​

Motion Collective Dance, Inc. is not responsible for lost, stolen, or damaged personal belongings brought to classes or events.

Agreement & Acknowledgment

​

By enrolling in a program, purchasing a membership, booking a class, or participating in any Motion Collective activity, I acknowledge that I have read, understood, and voluntarily agree to this Media Release & Liability Waiver.
